  1. How do I get access to the UC/Valley Oak claim system?
  2. You need to print a hard copy of the UC/VOS Access Form from the Valley Oak UC Support website at http://www.valleyoak.com/ucsupport/.

    • Type in your login and password.
    • Click on “Access and Security”.
    • Click on “Request for Access to the University/Valley Oak System”.
    • Print form.
    • Send a completed original form to Risk Services, 1111 Franklin St., 10th Floor, Oakland, CA 94607-5200. Be sure that the user requesting access has signed and agreed to the confidentiality statement at the bottom of the form and the supervisor has also approved and signed the form (new internal audit requirement).
  3. How do I delete access for someone from the claim system?
  4. You need to print a hard copy of the UC/VOS Access Form from the Valley Oak UC Support website at http://www.valleyoak.com/ucsupport/.

    • Type in your login and password.
    • Click on “Access and Security”.
    • Click on “Request for Access to the University/Valley Oak System”.
    • Print form.
    • Check the “Delete” section at the top of the form and fill out the name of the person to be deleted and which programs they are to be deleted from. Fax the form to UC Risk Services at 510-587-6481.

    • Valley Oak and/or Citrix - You need to print a hard copy of the UC/VOS Access Form from the Valley Oak UC Support website.
      • Type in your login and password.
      • Click on “Passwords”.
      • Click on “Password Change Form/Reactivate to the UC/VOS”.
      • Print form.
      • Complete the information and sign the form. Fax the form to UC Risk Services at 510-587-6481.
    • Aonline – Call the Aonline Help Desk at 1-800-557-6670 between 8:00 – 8:00 eastern standard time.

    • Go to Aonline.
    • Type in your user name and password.
    • Click on “Policies”.
    • Click on “Office of Risk Management Reference Material”.

  1. How do I get the insurance summary for a specific fiscal year?
    • Go to Aonline.
    • Type in your user name and password.
    • Click on “Policies”.
    • Click on insurance summary for the applicable fiscal year.
