The form is available in two formats:

• As a WORD document which may be opened, completed and saved on your local workstation. You may also wish to save the form on your local workstation BEFORE you complete it so that you can use it to submit future waiver exceptions. If you have problems opening and viewing the WORD document, please try the PDF version.

• As an interactive PDF document which requires that you have installed on your local workstation the free Adobe Acrobat Reader. This software will allow you to view, complete on screen, and print the document. You can also save this file on your workstation as a blank document. When opened in Netscape, it will be downloaded to a "temporary" directory on your workstation, but if you move it to a place where you keep your documents, it will be available for future use.
