Subject: CANCELLED Interagency Master Agreement No. 88-94812 A-4, California State Department of Health Services - Division of Environmental and Occupational Disease Control
This memo transmits a copy of the subject amendment, which extends the end date of this Master Agreement to June 30, 1994, transfer state program responsibility to the Division of Environmental and Occupational Disease Control from the Hazardous Materials Laboratory, and provides a new address and contact for invoicing purposes.

INTERAGENCY AGREEMENT
NUMBER 88-94812 A-6
THIS AGREEMENT is entered into this 31st day of December, 1993 by and between the undersigned State Agencies:
I. In that certain agreement between the State Department of Health Services and the Regents of the University of California, dated March 1, 1989, and amended on April 15, 1991, June 26, 1992, and June 1, 1993:
A. Paragraph 2 is amended to read:
"2. Purpose
This Interagency Master Agreement (IMA) is between the Regents of the University of California, hereafter known as UC, and the Department of Health Services, Division of Environmental and Occupational Disease Control, hereafter known as Agency. UC will perform services including, but not limited to, educational, consulting and/or research service in response to written requests from Agency."
B. Paragraph 6 is amended to read:
"6. Term of Master Agreement
The term of this Agreement shall be from March 1, 1989 through June 30, 1994."
C. Paragraph 7.A. is amended to read:
"7. Master Agreement
A. Contract Managers

D. Paragraph 11 is amended to read: "11. Invoices
Invoices in triplicate shall reference this IMA and the applicable MOU, and the UC campus Accounting Office address to which payment shall be sent, and include direct expenses and the overhead rate consistent with the elements in the budget approved by the Agency.

E. Attachment A, I. is amended to read:
"Carry out specific health effects studies requested by the Division of Environmental and Occupational Disease Control as follows:
Studies concerned with the occurrence, fate, and transport, treatment and environmental health effects of hazardous materials. $690,000."
II. The effective date of this Amendment is December 31, 1993.
III. All other terms and provisions of said agreement shall remain in full force and effect.
