Service Description
The collection of personal information from individuals is subject to various state and federal regulations. Thus, UC forms that request an individual's information, such as birth date, home address, and social security number, may be required to carry a privacy statement indicating how the personal information will be used. New forms, and forms that are being revised or reprinted, should be submitted to IR&C Records Management Services for a compliance review.
