MEDICAL EDUCATION AND TRAINING PROGRAMS

  • The UCD Medical Center serves as the clinical base for the School of Medicine and a wide range of education, research, and clinical training programs.

  • The UCD School of Medicine ranks among the top schools nationwide in the graduation of students committed to careers as generalists. UCD annually trains approximately 385 medical students. For many years, more than half of the students in UCD's graduating medical school class have entered primary care fields.

  • UCD provides training to approximately 700 medical residents in a wide range of hospital and community settings located throughout northern California. Among these, approximately 55% of UCD residents are training in primary care specialties, with nearly 30% of all positions in family practice.

  • UCD has a national reputation as a leader in the training of primary care medical residents, particularly in its rurally-based, network of affiliated family practice programs.
    PATIENT CARE SERVICES
  • UCD's clinical enterprise includes an acute care hospital, and an extensive network of affiliated community hospitals, clinics, and physician group practices throughout the Sacramento region. The Medical Center serves as the dominant Level I trauma center of the two in northern California. The medical staff consists of over 1,000 primary care and specialty physicians, many of whom have gained national reputations for excellence.

  • UCD Medical Center provided 124,325 days of hospital care and 555,949 clinic and emergency visits in FY 1995-96.

  • In FY 1995-96, 66% of UCD's hospital patient days were paid by the federal Medicare (24%) and Medicaid (42%) programs. Only 21% of patient days were covered by private payers, including managed care (20%) and fee-for-service (1%) plans. A significant percentage of patient days (13%) were covered by public programs, primarily County, or were self-pay.

  • UCD provides over 50% of Medicaid inpatient services in Sacramento County. UCD is also the major provider of specialty care to Medicaid beneficiaries in northern California, outside the San Francisco Bay area.

  • UCD is the largest provider to the uninsured and underinsured in Sacramento County, largely as a result of its contract with the County to provide emergency room and other clinical care to medically indigent patients.
PATIENT CARE REVENUES
  • Among total net patient revenues in FY 1995-96, 47% came from private payers, including managed care (43%) and fee-for-service (4%) plans. Federal program revenue represented 41%, with Medicare at 26% and Medicaid at 15%. The remaining revenue (12%) was from public programs, primarily the County contract, and from uninsured/self-pay patients.

  • In FY 1995-96, UCD was reimbursed $77 million less than the costs of services for Medicaid beneficiaries. Including Medicaid losses, UCD provided over $85 million in uncompensated care to low- income patients.

  • In FY 1995-96, Medicare's Disproportionate Share Hospital (DSH) reimbursed UCD $18.3 million for care to its disproportionate share of medically indigent patients, including those covered by Medicaid.

  • In FY 1995-96, Medicare's Indirect Medical Education (IME) adjustment provided UCD $26.2 million as compensation for the severity of patient illnesses, scope of services provided, and other indirect costs incurred related to medical student and resident education.

  • In FY 1995-96, Medicare's Direct Medical Education (DME) payments to UCD were $3.5 million to cover a portion of the costs associated with resident salaries and faculty teaching and supervision. Only Medicare (IME and DME) pays for costs related to medical education.

  • Medicare's medical education and indigent payments made up 10.5.% of UCD's total net revenues.
NOTABLE ISSUES
  • In response to a very competitive regional market in its primary service area, UCD has formed an alliance with major community hospitals and the Mercy Hospital network in its region.

  • UCD is extremely vulnerable to State and federally imposed reductions in disproportionate share funds for Medicare ($18.3 million) and for Medicaid ($11.7 million).

  • As a demonstration county for Medicaid managed care, all AFDC beneficiaries in Sacramento County have moved to managed care in the last 18 months. Rapid growth for Medicare HMOs is also occurring in the Sacramento region. UCD is addressing the dilemma of losing traditional Medicaid and Medicare patients to competitors, while receiving lower payments for many patients who are retained.
