Behavioral Health Benefits
UC's non-Medicare medical benefits include behavioral health benefits, which are provided by United Behavioral Health (UBH). These benefits are also available to Medicare members of Anthem Blue Cross PLUS and Anthem Blue Cross PPO.

The Medical Plan Chooser is an interactive tool that allows you to compare the benefits of UC-sponsored Medicare or Non-Medicare medical plans available to you, based on your zipcode.

Moving out of a plan's service area

If you move out of your plan's service area, you can transfer to a UC-sponsored plan that provides service in your new location. You must transfer within 31 days of the date you leave the service area. Contact UC Customer Service at 1-800-888-8267.

You and/or your eligible family members may also need to select a new primary care physician. Contact your medical plan carrier.
