Open Enrollment starts 8 a.m., October 31 and ends 5 p.m., November 22, 2012.

Because you elected to continue your University of California medical, dental, vision and/or wellness plans under the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA), you can make changes during UC’s Open Enrollment, with changes effective January 1, 2012.

This year, CONEXIS, UC’s COBRA administrator, will mail Open Enrollment information, 2012 rates and an enrollment form to all UC COBRA participants eligible to continue COBRA coverage on January 1, 2012.

You may make Open Enrollment changes via the CONEXIS website or by completing the enrollment form and returning it to CONEXIS.

Other Required Forms for Medicare-eligible Continuants
If you are transferring into or out of an HMO medical plan for Medicare enrollees you will need to call your new plan for additional Medicare requirements.
