[November 12, 1999]

If you are considering making an Open Enrollment change this year, you need to act now. Open Enrollment ends at midnight on Monday, November 22. No changes can be made after that time.

The following FAQs will help guide you through the process.

  1. Where can I find Primary Care Physician (PCP) Codes?
  2. Our website has online plan directories. Because these directories are not updated daily, you should call the plan directly if you are concerned about whether a particular physician is in the plan

For most plans you can find PCP Codes—as well as a wealth of other information, like physician profiles, satisfaction surveys, formulary comparisons, and consumer information—on the Find a doctor website. Health Pages gets its information directly from the medical plans and is still in the process of establishing relationships with some of our plans. Some plans, including UC Care, have not yet provided PCP information to Health Pages.

  3. How do I make an Open Enrollment change?
  4. Employees: You will make your changes by calling the Open Enrollment Action Line, our automated telephone system. First complete the Open Enrollment Worksheet (California or Los Alamos) then call the Action Line at the phone number shown on your worksheet. You'll need your Open Enrollment Code (shown on your Open Enrollment statement) or your Benefits PIN to access the Action Line. If you are changing medical plans, you will also need Primary Care Physician (PCP) Codes (see #1 above) for each person you are enrolling. PCP Codes are not needed for Kaiser, Core, High Option, or BluePremier worldwide benefits.

Annuitants: You will make your changes by submitting an Open Enrollment Change Form. If you are changing medical plans, you will also need Primary Care Physician (PCP) Codes (see #1 above) for each person you are enrolling. PCP Codes are not needed for Kaiser, Core, High Option, or BluePremier worldwide benefits. You may also need a Medicare disenrollment form. Please read the Open Enrollment materials (California or Los Alamos) for details.

  5. Can I make an Open Enrollment change on this website?
  6. No. This website contains lots of useful information about Open Enrollment, but it cannot be used to make Open Enrollment changes. Employees must make their changes by calling the Open Enrollment Action Line, an automated telephone system. Annuitants must make their changes by submitting an Open Enrollment Change Form.

  7. If I enroll in the legal plan, family coverage, do I have to enroll my eligible family members or are they enrolled automatically?
  8. You must enroll them.
