[September 9, 1999 ]

Effective October 1, 1999, the Brown and Toland Medical Group in San Mateo will no longer be a participating provider within the PacifiCare/Secure Horizons network. PacifiCare will begin transferring members assigned to this medical group to alternative participating providers beginning September 15, 1999.

In some cases, members will be able to remain with their current primary care physician through their physician's affiliation with Brown and Toland Medical Group/San Francisco or Mills Peninsula Medical Group.

All medical benefits and services through PacifiCare will remain the same and members will continue to receive medical services without interruption.

Up until September 30, 1999, members should continue to receive medical care from their Brown and Toland Medical Group/San Mateo primary care physician or the specialist to whom that physician has referred them. Starting October 1, 1999, except in emergency situations, medical care should be provided or arranged by their new contracted medical provider.

On September 1, PacifiCare sent a letter to affected members which explains the details and options regarding this change. Members with questions should call PacifiCare's Member Services Department weekdays between 8:00 a.m. and 8:00 p.m. at 1-800-624-8822.
