[March 25, 1999]

Tax Verification Requests are being mailed this week to 3,172 University of California employees and annuitants, asking them to document tax dependency for approximately 4,000 family members.

Employees and annuitants will receive the mailing if their UC-sponsored medical, dental, vision, or legal coverage in 1998 included at least one of the following family members:

  • a child under age 23 who is not the employee/annuitant's natural or adopted child;
  • a disabled child age 23 or older;
  • an adult dependent relative; or
  • a same-sex domestic partner whom the employee/annuitant claimed as tax dependent.

The response deadline for all who receive a Tax Verification Request is Friday, April 30, 1999. Those unable to submit documentation by this date—for example, if they have applied for an extension from the IRS—must respond nevertheless by the April 30, 1999, deadline, alerting Insurance Services at UC HR/Benefits that they need additional time.

Those who do not respond will be notified that the children are deemed ineligible and will be de-enrolled retroactively to January 1, 1998. Insurance carriers will also be advised.
