[March 6, 2001] Requests for tax verification are being mailed this week to 3,530 University of California employees and annuitants, asking them to document tax dependency for 4,378 family members. Employees and annuitants will receive the mailing if their UC-sponsored medical, dental, vision or legal coverage in 2000 included at least one of the following family members:
The response deadline for those who receive a Tax Verification Request is April 15, 2001. Those unable to submit documentation by this datefor example, if they have applied for an extension from the IRSmust by the April 15, 2001 deadline, notify the Health and Welfare Administration at UC HR/Benefits that they need additional time. Those who do not respond by the deadline will be notified that the children are deemed ineligible and will be de-enrolled. Insurance carriers will also be advised.
